Tomorrow the neurosurgeons and plastic surgeons are planning another surgery. This will be for a few reasons. First is the wound closing. They have been doing stitch after stitch. That is working (in addition to the extra protein nutrition they are giving him) to give time for the wound to heal more. But, it's not enough. Now they just want to fix it totally. As was explained to me, if something can go out (i.e., leaking from the wound), then something (i.e., infection, germs) can go in. They are hoping to just have to clean out the necrotic tissue and suture the wound. If that's not enough, they will use a local flap, meaning that they will move skin from his head to cover the brain and then take a skin graft from another part of his body. If that's not enough, they will take a flap from somewhere else and hook it up with the skin on his head. The problem is that he doesn't have a skull (bone) covering his brain and therefore they need to put skin/tissue over the brain that can grow and protect the brain.
Now for the neurosurgery team: They don't know what's in the brain. They will do a CT scan today that will tell them more information. They will know whether to suspect an abscess or if there is more necrotic tissue in the brain. As long as they are in the brain, they will try to clean out what they can do safely. There is a risk of additional brain damage, but it will be less than if they wait and do nothing. If they think they need, and it is a good possibility, they will insert another drain.
So, for now it is a wait and see and pray.
